Loss of muscle strength, mass and quality is expected as part of the natural ageing process. Sarcopenia is a progressive, accelerated loss of muscle, accompanied by low physical activity.1 Without a nutritional or exercise intervention muscle loss is inevitable following prolonged bed rest, whether it be due to age, extended illness from winter colds and flus, obesity or from chronic conditions. A significant decline in the rates of skeletal muscle synthesis is thought to be responsible for this muscle atrophy.2

Loss of muscle mass mostly affects muscles of the lower body and occurs most rapidly in the first few days and weeks of inactivity.2 In one study, healthy young adults subjected to bed rest lost lower body lean mass at a rate of 1-200g per week.2 Another study of young women reported a 21% (quadriceps) and 29% (triceps) decrease in muscle volume after 60 days of bed rest.2 The losses in older people are much greater. In one group of healthy older adults, there was a 950g loss of lean leg mass after 10 days of bed rest.2 This equates to a three to six-fold greater loss than in younger people.2 Resistance exercise can provide anabolic stimulus, but this is not always possible.3

Beta-hydroxy-beta-methylbutyrate (HMB) is a metabolite of leucine. HMB works via several mechanisms (see Figure 1). Firstly, it increases protein synthesis via mammalian target of rapamycin (mTOR) and by stimulating the production of Insulin-like Growth Factor-1 (IGF1). Secondly, it reduces proteolysis through the inhibition of the ubiquitine-proteasome system (UPS). The UPS is a crucial protein degradation system in human cells. Thirdly, HMB increases the proliferation of satellite cells by the stimulation of Myogenic regulatory factors (MRFs).4 MRFs regulate myogenesis, the formation of muscular tissue.

Supports muscle health and function

HMB was supplemented in a prospective, randomised, double blind, placebo-controlled trial involving 24 healthy older adults. They were on bed rest for 10 days and then followed a resistance exercise program for eight weeks. They were also given 1.5g of HMB twice daily starting five days before bed rest until the end of the rehabilitation period.3 There was no significant difference between groups at baseline. Lean body mass remained stable in the HMB group compared to the placebo group despite bed rest.3 Many even experienced gains in leg muscle mass and strength above baseline, attributed to the combined effect of HMB and exercise.3

Creatine is found in animal protein and is also endogenously produced by the liver, kidneys and pancreas from the amino acids glycine, methionine and arginine.5 Creatine is involved in a reaction that rapidly replenishes ATP from ADP and phosphocreatine.6 It is found in cells with high and fluctuating energy demand, such as skeletal muscle and the heart. Creatine plays a role in the shuttle of high-energy phosphates between the mitochondrial sites of ATP production and the cytosolic sites of ATP use.6

Three groups of male university students were given either 3g or 5g of creatine, or placebo for 35 days. At endpoint, the percentages of muscle strength increases were significantly higher in the intervention groups compared to placebo; and upper limb muscle endurance was greater in the 5g group.5

Maintains muscle mass in ageing individuals

Maintaining hip and leg strength supports mobility, independence and reduces the risk of falls.7 A group of men aged in their 60s were given 3g HMB or placebo daily for 6 weeks, alongside resistance exercise training. HMB increased thigh lean muscle mass and chronic muscle protein synthesis.8

Creatine plus resistance training improved appendicular lean mass and muscle function in a group of older women over 24 weeks.9

Supports muscle strength during resistance training

Specific training, diet and supplementation for athletes is aimed at increasing performance by aiding recovery and improving energy. Combined 3g HMB daily and 0.04g/kg/day creatine monohydrate showed a synergistic effect on aerobic power in a group of endurance athletes.10
