There are several different forms of folates, including more than one active form. The most biologically active forms are Folinic Acid and methyltetrahydrofolate (MTHF), with the latter being the final step in the metabolism of folate (or the synthetic form, folic acid). However, the physiologic steps to convert folate into MTHF are numerous, and certain genetic predispositions can impede folate conversion. Folinic Acid offers an active form of folate, bypassing several metabolic steps towards MTHF production.

Folate Metabolism

Folate occurs naturally as a complex of related substances – folates – that are found in foods.18 However, food processing and preparation can destroy almost all of the naturally occurring folate, as it is highly sensitive to heat, air and light.17 Oral folates are generally available in two supplemental forms: folic acid and Folinic Acid. Although the most common supplemental form of the folates is folic acid, it only makes up 10% or less of dietary folates. The majority of folates in the diet consists of reduced folates and methyltetrahydrofolates. While folic acid is generally well absorbed, evidence suggests that reduced folates and methyltetrahydrofolates are absorbed differently.

Both folate and the synthetic folic acid are ultimately converted into methyltetrahydrofolate, with a number of steps in between. Folate must be converted to dihydrofolate (DHF), then to tetrahydrofolate (THF), methylenetetrahydrofolate (METHF), and finally, to methyltetrahydrofolate (MTHF) (Figure 1). Folinic Acid bypasses the oxidation and reductions steps required to convert folate to THF.2

Absorption and utilisation of folates can be impeded by a number of factors, including a nutrient deficient diet or digestive issues. Genetic polymorphisms of a number of enzymes, including dihydrofolate reductase (DHFR) and methylenetetrahydrofolate reductate (MTHFR), can also impact folate metabolism.4, 5 DHFR converts folate to dihydrofolate. Polymorphisms of this enzyme, or inadequate levels of vitamin B3, reduce the activity of the DHFR enzymes, leading to a decrease in tetrahydrofolate (THF).

MTHFR is required for the conversion of METHF to MTHF. Polymorphisms of the MTHFR gene are the most well studied genetic influence on folate activity and can affect the activity of the enzyme. Extreme MTHFR deficiency is very rare, with only about 100 cases being reported worldwide.6

In the presence of sufficient folate derivatives, such as Folinic Acid, conversion of MRTHF to MTHF does occur.7

Folinic Acid

Folinic Acid, or formyl tetrahydrofolate (formyl-THF) is a more active form of folate, and as such, bypasses several steps in the conversion of folate to MTHF – the molecule the body uses in methylation reactions.

Following an oral dose of Folinic Acid, the majority of folates are metabolized to 5-MTHF directly during absorption in the intestine, bypassing the need for deconjugation and subsequent reduction in the liver.11 The rate of conversion of Folinic Acid to MTHF in the intestine is such that greater absorption and conversion is gained from oral doses than from IV administration.12
